What is Advance Care Planning? An Advance Directive? A Healthcare Agent? A POLST form? You are invited to a workshop on completing an Advance Directive. You will be guided through the process of choosing a trusted decision maker and putting your wishes in writing related to healthcare in the event that you are unable to speak for yourself. All documents will be provided, and a Notary Public will be available to notarize your completed document if desired. Hosted by the PeaceHealth Advance Care Planning Program. Duration of presentation: Approx. 90 minutes, with Q & A at the end. Open to PeaceHealth patients and all members of the community.
