Advance Care Planning Workshop

What is Advance Care Planning? An Advance Directive? A Healthcare Agent? A POLST form? You will be guided through the process of choosing a trusted decision maker and putting your wishes in writing related to healthcare if you are unable to speak for yourself.
Open to PeaceHealth patients and all members of the community. The session will be conducted in the cafeteria.
Monthly sessions begin February 19th and continue the 3rd
Wednesday of each month in 2025. Registration encouraged but not
required. Bring your questions and a photo ID for notary if you wish to
complete a document during the workshop.
