Advanced Care Planning Workshop 2nd Wednesday of the month, April 9, 3:00 pm - 5:00 pm PeaceHealth Cordata Parkway. What is Advance Care Planning? An Advance Directive? A Healthcare Agent? A POLST form? You will be guided through the process of choosing a trusted decision maker and putting your wishes in writing related to healthcare if you are unable to speak for yourself. Session held at Cordata Main- Lower Level Conference Room.
