Advanced Care Planning Workshop

Advance Care Planning Workshop 3rd Wednesday of the month, September 18, 3pm - 5pm PeaceHealth United General Medical Center Sedro Woolley Campus.

What is Advance Care Planning? An Advance Directive? A Healthcare Agent? A POLST form?  You will be guided through the process of choosing a trusted decision maker and putting your wishes in writing related to healthcare if you are unable to speak for yourself.

Open to PeaceHealth patients and all members of the community. The session will be conducted in the cafeteria.
