Advanced Care Planning Workshop 2nd Wednesday of the month, March 12, 3 p.m. - 5 p.m. PeaceHealth Cordata Parkway. What is Advance Care Planning? An Advance Directive? A Healthcare Agent? A POLST form? You will be guided through the process of choosing a trusted decision maker and putting your wishes in writing related to healthcare if you are unable to speak for yourself. Session held at Cordata Main- Lower Level Conference Room.
