Behavioral Health Clinician

This position is part of a multidiscipline care management team that provides ongoing, individual counseling to a diverse adult population in their place of residence including psychotherapy, treatment planning, identification and development of natural supports, advocacy, and collaboration with other supports. 

Care Coordinator

Assists our highest-need clients, providing ongoing support to aid them in achieving established health and recovery goals through care coordination, comprehensive case management utilizing health screening / assessment tools, health education, and evidence-based practices.
