An informational interview is a brief, informal meeting with a professional in the field you are interested in. The intention is to gather information, insight, and advice about their career and day-to-day job duties. Informational interviews are not job interviews and are not connected to the formal recruitment & selection process.

Positions
Care Coordination+
Care Coordinators are based primarily in the field while providing both intensive care coordination and comprehensive care management. Care Coordinators provide coordination of medical and social services needed to clients such as, long-term care, transportation, food services, mental health advocacy, medical specialist and referral appointments; while facilitating communication between the clients medical providers, family, and other caregivers.
Case Management+
Long-term Care Case Managers assist clients in developing a plan of care that lets them live at home with as much health and safety as possible. NWRC works with those who choose to live in their own homes rather than move into a nursing home, assisted living, or adult family home. Our case managers will coordinate and help get the needed services, which include caregiving, nursing, meals, medical equipment, and more.
Behavioral Health+
Behavioral Health Clinicians travel to clients homes across the four-county region to provide personalized care and psychotherapy to our clients, which amounts to 20 hours per week of billable services (approximately 4 sessions per day). NWRC clients often receive services from multiple programs across NWRC. This allows clinicians to engage and collaborate with other social services professionals at NWRC to build a comprehensive care plan which truly places the client at the center.
Aging & Disability Resources+
Aging & Disability Resources Program Specialists provide information regarding services, products, and/or resources available to clients and their families through social service agencies, medical providers, and community partners in the aging and disability network. ADR Specialists screen clients for services, make referrals to the appropriate resource(s) and provide general assistance to clients and families.
Non-Emergency Medicaid Transportation+
NEMT Customer Service Representatives screen clients for eligibility to access the Medicaid Transportation Program and work with clients and medical providers to assess client’s physical capabilities and current mobility status. The NEMT Customer Service Representative assigns transportation to the lowest-cost, most appropriate transportation provider and maintains trip-related information in the computer database.
Program Administrative Support+
Case Aides provide program support within the NWRC Care Management, Clinical Programs, and/or Community Programs Units. This position assists supervisory and field staff as well as subcontractors in elements of service delivery.
